New Patient Registration

Please call into any of our branches to collect and complete a New Patient Registration form. One will need to be completed for each member of a family joining the practice. At this time you will be offered the opportunity to make an appointment with our healthcare assistant. This helps us to ensure we have a brief back ground to your health while waiting for your records to arrive from your last practice. This can be important for those who are on repeat medication to ensure there is no delay in issuing further prescriptions.  If you are aware that you will need medication in the near future, please make an appointment to be seen by a GP at this time also to avoid running out.

For us to be able to retrieve your medical records from your previous GP please contact them for your NHS number and have this ready with your form at your appointment with the healthcare assistant.

Although patients now register with the Practice and not an individual GP, you will still retain the choice, where appropriate, to request to see an individual GP.   If you have any queries regarding this please call in or speak to a member of staff on 01443 490366.

Change of Contact Details

Please notify us of any changes to your address, name and telephone number. Please note that if you are awaiting an appointment from a local hospital, please inform the hospital of your change of details also.  Incorrect details can mean a delay in your treatment.
